8/13/2023 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, Ashiyah appeared to be in a positive mood evident by his facial expressions such as a smile on the face. QMHP-C-C engaged Ashiyah in a discussion to determine her behaviors since the last session. She verbalized to QMHP-C-C she don’t be communicating with her mother her thoughts and feelings. Behavior: Ashiyah appeared to be motivated by positive encouragement while being praised for her hard work. Ashiyah said thank you and prepared to answer questions regarding her mental health and wellness but at times within the session she was prompted for displaying off task behaviors as she was engaging in activity on her phone. Intervention: QMHP-C had one on one with Ashiyah. QMHP-C went over services, objectives, and goals. QMHP-C processed Ashiyah’s thoughts and feelings. QMHP-C commended Ashiyah on utilizing coping strategies to increase her understanding of feelings and emotions. QMHP-C assisted Ashiyah in monitoring her own behaviors in efforts to promote exercising self-control in focused areas such as respecting boundaries with others. QMHP-C inquired about evening routine. Intervention: Today’s interventions were focused on objectives 1, 2, and 3. QMHP-C discussed with mom about Ashiyah’s involvement during their session and how much improvement is noticed so far. QMHP-C provided simple non-verbal praises as Ashiyah was observed displaying positive home behaviors. QMHP-C explored positive behavior options she was exhibiting with Ashiyah to support her willingness to continue to make good decisions and display appropriate behaviors in the home. Response: Ashiyah joined session to utilize and learn new interventions and strategies. Ashiyah expressed appreciation to QMHP-C by smiling and giving good eye contact. Ashiyah discussed mental illness and its causes as a family. Ashiyah was receptive to the recommendations that she received from her counselor as evidenced by her providing active feedback and communication during the therapy conversation. Ashiyah demonstrates leadership skills by helping others in the home. Response: Ashiyah displayed interest in learning about ADHD. Ashiyah has become more involved in age appropriate social activities and was given positive reinforcement for these choices. Ashiyah was receptive to the interventions, as evidenced by her cheerful expressions. Ashiyah displayed high ambition and was very productive, despite distractions. Ashiyah communicated being home was a positive place for her compared to her out of home environment.
